Shearwater Systems has developed the Rapport Clinical Portal and the Rapport Patient Portal to provide mobile access to Graphnet’s CareCentric product suite.
Developed and sold by Graphnet Health, CareCentric combines data from large numbers of systems and organisations into a single electronic health and care record.
The Rapport portals sit on top of CareCentric and open up this rich record to authorised users on smartphones and tablets. The portals give them access to patient data wherever they need it, whether at various locations within a hospital or GP practice, at other hospitals, in the community or at home.
CareCentric is one of the country’s leading electronic health record products. It holds data on more than 17m patients. It is used by over 30,000 care professionals and provides secure electronic patient records across hospitals, mental health trusts, social services departments and whole local health economies.
In Hampshire, for example, CareCentric is used to combine data from 157 different systems, spanning 9 organisations and 142 GP practices. Poole Hospital NHS Foundation Trust has used the CareCentric product suite to implement an electronic patient record system which is tailored specifically to meet its needs, and which is used by over 5,000 users at the hospital itself and in clinical networks, GP practices and social services across Dorset.
CareCentric connects to most of the major health and social care IT systems being used in the UK, including the majority of GP systems (EMIS, iSoft, INPS, Microtest etc).
