Your data is stored on Microsoft® HealthVault™ secure servers.

You may request details of personal information which we hold about you under the Data Protection Act 1998. You may request access to your medical records we hold about you under the Access to Health Records Act 1990.

We require Personal Data to understand your needs and provide you with a better service.
Sensitive Personal Data is collected so that it will be available, as necessary and in a timely way, to enable members of your care team to safely and effectively treat and care for you. It will only be used with your consent.
